Bank Requirements
STANDARD APPLICATION DOCUMENTS
1. Completely filled out application form.
2. Latest ID photo of borrower and spouse - 2pcs each
3. Photocopy of 2 valid Gov't. ID's w/ photo and signature. (eg. Driver's license, SSS and Passport)
4. Latest Income Tax Return (ITR)
5. Photocopy of Marriage Contract/ Birth Certificate
6. TIN ID
7. Cedula
IF LOCALLY EMPLOYED
1. Original copy of Certificate of Employment, indicating length of service, position and annual salary.
2. Photocopy of payslips for the past three (3) months
3. Photocopy of the Statement of bank accounts for the last six (6) months OR photocopy of passbook for the past six (6) months.
IF EMPLOYED ABROAD
- For seamen, latest crew contract and Employment history.
- For nurses, contact workers and immigrants - employment contract or Certification of employment & salary
- Proof of monthly remittances OR payslips for the past three (3) months.
- Photocopy of the Statement of bank accounts for the last six (6) months OR photocopy of passbook for the past six (6) months.
- Special Power of Attorney -Bank Format to be consularized by the Philippine Consular Office
- Special Power of Attorney -Harvard Format to be consularized by the Philippine Consular Office
- Photocopy of Passport
- 2 Valid Gov't. IDs and Cedula of AIF/ Representative
Important reminder for SPA's
- If executed abroad, it should be authenticated or consularized by the Phil. Consulate
- If locally executed during client's vacation, it should be notarized here.
IF SELF-EMPLOYED OR APPLICANT IS IN BUSINESS
- Latest Income Tax Return (ITR)
- Audited Financial Statements for the last 2 years.
- Certificate of Business Registration with DTI or SEC Business Registration & By-laws, whichever is applicable.
- Photocopy of the Statement of bank accounts for the last six (6) months OR photocopy of passbook for the past six (6) months.
- Business Background / Company Profile
- List of Suppliers/ Customers at least 3 for each with contact persons and telephone numbers.
Note: Additional documents may be required to facilitate the processing of the loan application.
